Trip Forms – Once travel arrangements are booked, clients will be given the opportunity to submit their required personal information via our Trip Forms. Although different information is required for different destinations, the Trip Forms can ask for the following: payment information, emergency contact information, proof of medical evacuation insurance, past medical history, current medications, physical limitations, dietary preferences, passport information, arrival information, departure information, and a Release of Claims.

Use of Information
We may use personal information to provide the services you’ve requested, including services that display customized Trip Forms. We may also use personal information for auditing, research and analysis to operate and improve Asia360° Travel Co. We may use third parties to assist us in processing your personal information, and we require that such third parties comply with our Privacy Policy and any appropriate confidentiality and security measures.
